I have read the following information about the processing of my personal data.
If you fill in the booking form, remember that:

2. The administrator will process your personal data provided in the booking form in order to take action on you at your request, before concluding the contract.
Legal basis: art. 6 par. 1 point b) of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (RODO) .
3. Your personal data The Administrator will keep until the completion of activities undertaken at your request in accordance with your request contained in the booking form.

4. The recipients of your data will be entities implementing, at the request of the Data Administrator, IT services, including the server owner, on which the Data Administrator's website is located.

5. You have the right to request access to the content of your data and the right to rectify it, delete it, and from 25/05/2018 the right to obtain a copy of the data, data transfer and the right to demand limitation of data processing.

6. If you feel that the processing of personal data regarding you violates the provisions of the Act on the Protection of Personal Data, or the GDPR, you have the right to lodge a complaint with the supervisory authority competent to hear this complaint on the date of its submission.

7. Providing personal data by you is voluntary, but failure to do so will prevent the Administrator from taking action at your request indicated in the booking form.
